sick tang

I have a blue tang that I've had for about a month now. In the last 2 days it has stopped eating and is looking really thin. I noticed ich on it a day ago but the cleaner shrimp seems to have taken care of that. Today it is just lying on the bottom and occasionaly getting stuck on the filter intake. Any ideas? Am I going to lose this fish?

Any help would be great, thanks
Sheila
