What can make your tank crash?

Hello... I hope everyone had a good holiday.

No my tank has not crashed but I'm working on a presentation and I wanted your input.

What do you think are top 10 reasons peoples tanks crash.

Too much (livestock/coral/inverts/etc), too fast..
Using strait tap water to top off.

Most of the crashes that I've known about were caused by...

Power failure
Heater or Chiller malfunction
Auto top off malfunction (fills tank with freshwater)
Kalkwasser overdose
Disturbing a mature sandbed

Not realizing SW/Reefing is far less forgiving than FW.
Going on vacation, and forgetting your house sitter is not a reefer.

Toxic additives.

You would be surprised what people put in their tanks without first either researching what it is and how it should be used or if there is no non-biased information then not adding it.

Just some examples:

1) Copper to a reef tank with inverts

2) Adding too much phosphate adsorbant materials too quickly to a reef tank and shocking the inverts (either with the sudden change in phosphate levels or alk).

etc.
