Proper Technique for Formalin Bath

Can someone please describe how to properly conduct a formalin bath? I've read how on petforums.com but I am having trouble understanding the instructions. Can someone with formalin use experience describe it in lazyman terms how to set it up, how it should be monitored, and how to dispose of the formalin afterwards?
