sick fish, need help

breathing is normal, just scratches alot

the clear outer part of the eye looks swollen, I guess internal, but only because it doesn't look like there is anything on the outside of the eye like a cyst or something. I think he has scratched it on the sand, but I'm not positive

Going to need more information to be able to give you any meaningfull help.

What is the fish? How long have you had it? What are the other tank mates? Any recent additions or changes made to the tank? What are the water test parameters? How frequently is it scratching? And any other info you can think to add.

Niger trigger, first fish in a new tank that cycled for 5 weeks using the dead shrimp method.

yesterday was scratching about 20 times in 10 minutes.
I didn't see him today, I got home after his bed time.
My wife said he wasn't scratching as much as yesterday.
