filter bags, how often to replace/clean ?

my filter bag seems 'clogged' and it is overflowing over the top...

do I need to replace this, or can I clean it with a hose or something ?

I just clean mine out in hot water and scrub it down until it is fairly white again....I will do this for awhile getting plenty of use out of it and then replace it...some actually wash it in bleach and then rinse well and but back in aquarium
