I am sure this is not a new syndrome but I am not sure how to deal with this. Garlic? leave it alone? Quarantine? Can someone lend me a hand? Thanks

If it's just one eye, I would hold off on medication for 4 or 5 more days. This is often due to a bump or scratch and usually heals itself. It wouldn't hurt to do a water change and/or soak it's food on Selcon or even Zoe to help the fish satay strong and healthy and cur quickly.

Sometimes this problem is caused by stray micro-bubbles from a protein skimmer or pump that is getting a regular air intake and output.

If the problem is in both eyes, there could be a water quality issue and a water change is more urgently needed in this case. Several may even be required.
This should be followed with a Saltwater anti-biotic that treats Pop Eye. Maracyn 2 for one example.
