That's a koran angel changing from juvenile to adult coloration. They get about a foot in length, have a lot of personality, do well in captivity, and contrary to what many will say, can be very friendly towards other fish. Reef-safe is pretty hit-or-miss with this species, I have one that bothers nothing, another that eats zoas, and I had one awhile back that annihilated an open brain. None of them have bothered any crabs, snails, or shrimp.

Hard to tell with such fuzzy pictures but it looks like a koran to me too.

Looks like a Koran to me too. However it also looks like it has HLLE, there appears to be a lot of missing scales on its face. What are you feeding it? Also improving the water quality should help too.

Here is an old picture of my Koran right before I sold it -- was getting to big for my tank.
