The first question I would ask is, where was the rock stored? Was it stored somewhere where it could have picked up impurities? If so, I would not use it at all.

Unfortunately, at this point, most of the life would be gone from the rock, IMO. It would, however, be usable for base rock.

Definatly. What makes the rock "live" is simply the bacteria on the rock. Now, the rock being out of water for this long, you would want to ensure that it had not been used in like yard landscaping or anythign where unwanted chemicals could have come into contact with it. Otherwise, just clean it well, and re-cure the rock (let the rock re-cycle with bacteria). It will re-seed with all sorts of life in no time at all.

Yes, but all that was live is now dead. So, you'll need to seed the rock again from another live rock. As suggested, such rock is often used for base rock. I've added such "dead" rock to my tank and after 6 months you can't tell the difference.

if you put the rock in a bucket or tub of saltwater with a peice of 'live rock' (out of your tank) that is already full of bacteria, and let it cycle with a powerhead (3-4 weeks) it will become live again (and over time you wont be able to tell the difference between it and other live rocks, good as new! lol)

live rock does not mean that the rock it self is alive live rock is when rock is filled with all little creatures and other benfical bacteria and stuff once that stuf is killed off it is considered dead. Its is reusable but you will need to get a couple of pieces of live rock to bring back the others once the coraline takes over you wont be able to tell the difference
