Help, dropsy on flame wrasse?

My tank has ich so i am currently doing hypo to the main tank, yes i know not good, lost life in live rock, etc..

When i reached 1.009 the ammonia was very high for two days, finally able to get it down to zero tonight with water changes and lots of amquel.

Before the ammonia went up my flame wrasse looked fine and ate. This is how he currently looks,

Basically like a pine cone if looked from the top, all scales sticking out. Do you think the reason for this was the high ammonia? Or secondary bacterial infection from the ich, although he was one of the few that showed no signs of ich.

thanks for your help!
