Ots

My tank is about 4 years old with 2-3 inches sand bed, many friends tell me to remove sand from the tank, they says about OTS.
I don't know the cause of OTS, can anyone tell me about this?
How is the signs of OTS?
How to prevent OTS?
Sea water or Salt Mix is better for prevent OTS?
