I think a skimmer is especially important for a FO because there are usually more fish in there, which creates more waste. Also, fish like puffers, who are ineffecient eaters, create much more waste...

How do you feed the photoplankton to your tanks?

I was thinking that if you just dump it in than your tank will have tap water in it. Also, is it neccesary to use a dechlorinator in the tap water before using it to culture the photoplankton?

Is it possable you have a test kit error? Most fish stores will do a free test for you, so you should have that done to be sure you have a problem.
