I'm so sorry your horse is having trouble. Please remember that I've never claimed to be an expert on disease and treatment. I would urge you to post in the emergency section at Seahorse.org for help. FWIW, here is my opinion. If he is swimming upside down, then he has gas under the skin. This could have resulted from an injury. Is it possible that his tail got snapped by something in your tank? Such as a clam or scallop? He could have some sort of infection resulting from an injury.
